This makes you wonder how many of the men bragging about the size of their genitals may be full of it. Full of penis filler or scrotal filler, that is. Apparently, it’s become a thing for men with moolah like finance bros to see doctors periodically to get such substances injected into their penises or scrotums to enhance their size and appearance. Just take a look at what helped fill Thursday night’s “Meanwhile” segment of The Late Show with Stephen Colbert:
As you can see, Colbert called this scrotum filler trend, “Yet another sad example of the pressure on men to live up to the well-known preference women have for huge scrotums. ‘Oh Rick is amazing. I mean he’s not tall or attractive or wealthy. But he’s got a scrotum the size of a duffle bag.’” If you didn’t notice, he was kind of injecting oh just a little bit of sarcasm into what he was saying.
Colbert didn’t just bag on the scrotum filler trend. He also stuck it to the penis filler trend after reading the following passage from a Business Insider article entitled “Forget male Botox — finance bros are lining up to get penis filler” and written by Julia Pugachevsky: “Bustamante [an aesthetic nurse practitioner who does such injections]
said he’s had clients ask him, while he was injecting them, if this is the most filler he’d ever used. When Bustamante said he’s gone higher, they’d ask him to match that number on the spot. Some clients have ended up with penises over 7.5 inches in circumference — greater than their length.” Colbert joked about how that could make your penis look more like a can of tuna. Ah, yes, you can.
What Is Penis Filler?
For those of you who have never added anything besides fun to your penis, a penis filler is a substance that’s injected into a penis to increase its girth. Like dermal fillers applied to other parts of the body, it’s injected just below the superficial layer of the dermis of the skin encasing the penis. Such injections can increase the circumference of a penis by one inch or more, depending on how much filler is injected.
While different substances can serve as filler, when it comes to non-permanent ones, hyaluronic acid is one of the most commonly used substances. Fat and collagen are options as well. You can reverse the effects of hyaluronic acid by an injection of hyaluronidase, an enzyme that can break down the bonds between hyaluronic acid molecules, allowing your body to go ha ha to the HA and metabolized it away.
What Is Scrotum Filler?
A scrotum filler is similar, except that you hit the sack instead. It goes into the dermal layer of the skin holding your testicles, affectionately known as your balls. Such an injection can make your scrotum look larger, fuller and heavier. This can include reducing the amount of wrinkles in your scrotum and the degree to which it sags.
As alluded to earlier, when hyaluronic acid is used as a filler, the effects don’t last forever. It’s a bit like a subscription service such as Microsoft 365, except its more like Penis 548 or Scrotum 730. If you want to maintain the resulting size or shape, you usually have to return in about 18 months or so to get your penis re-filled. Something like Scrofill, which is HA in, guess where, your scrotum, may last a little longer, like two to three years.
If you are looking for something that lasts much longer, like forever, you can get more permanent fillers such as polymethyl methacrylate in your penis or silicone in your scrotal sack. Plus, there are other procedures like scrotal tightening that I have discussed previously for Forbes. But that’s a whole different ballgame, so to speak.
What Are The Risks Of Penis Or Scrotum Filler?
As you can imagine, such injections are not completely without their risks. If a random person were to walk up to you at a coffee shop and say, “can I stick a needle in your penis,” your answer probably wouldn’t be “sure.” That’s because injections to your genitals in general aren’t fun. Any type of injection can result in infection, bleeding and scarring, and filler injections specifically can bring the risk of the filler accumulating in ways that you don’t want, such as causing nodules or some other type of deformity.
How common are such complications? Well, a study published in 2021 in the Asian Journal of Andrology found that such complications occurred in 4.3% of 230 patients who had received injections with hyaluronic acid into their penises from January 2018 to December 2019. Another study, published in 2024 in The Journal of Sexual Medicine, looked at 300 men who received 4,879 injections of UroFill over time (and not in one sitting.) None of them required removal of filler, eight of the injections resulted in localized skin infections that were treatable by antibiotics and the filler remained in place after 12 months 90% of the time.
It’s not clear whether such complication rates would hold across all patients everywhere. It’s also not clear what the complication rate for such injections into scrotums may be. HA was approved by the U.S.
The Food and Drug Administration approved the use of dermal fillers in 2003 for general purposes. However, more published studies are needed to specifically assess its efficacy for penile and scrotal enhancement.
The risk of complications associated with these injections can vary depending on the qualifications and experience of the injector. It is crucial to seek treatment from a board-certified plastic surgeon with a proven track record in performing such procedures. Do not rely solely on advertisements from unverified sources.
The cost of these injections can range from $3,000 to $15,000 per session, depending on the provider, type of filler used, and the amount injected.

Men opt for these procedures to enhance their self-image and confidence. While there is no medical necessity for increased penile or scrotal size, some believe that it may improve sexual satisfaction for their partners. However, the impact of size on sexual pleasure is subjective and complex.
Another motivation for seeking filler injections is the desire for a larger appearance, as some individuals associate size with superiority. It is important to consider the potential risks and consequences before undergoing such procedures.
It is essential to remember that self-esteem should not solely rely on physical appearance, as it is subject to change over time. Ultimately, true value lies in one’s character, values, and personality rather than external enhancements.
