Renowned rock musician Billy Joel, aged 76, has recently been diagnosed with normal pressure hydrocephalus (NPH), a neurological condition that has led to the cancellation of his upcoming tour. NPH, often mistaken for dementia, can significantly impact walking, balance, and cognitive abilities.

Understanding Normal Pressure Hydrocephalus
The brain consists of three main components: brain tissue, blood, and cerebrospinal fluid (CSF). CSF, a clear fluid, flows through the brain and spinal cord, providing cushioning and nutrients. In NPH, there is a dysfunction in CSF absorption, leading to fluid buildup in the brain’s ventricles. This excess fluid puts pressure on the surrounding brain tissue, causing symptoms that may mimic dementia but are distinct in nature.
NPH is relatively rare, affecting around 0.2% of individuals aged 70–80 and up to 6% of those over 80. While it is more common in older adults, its prevalence increases with age.
Causes of Normal Pressure Hydrocephalus
In NPH, the absorption of CSF is hindered, often due to dysfunction in the arachnoid granulations. These microscopic structures are responsible for reabsorbing spinal fluid back into the venous system. Unlike other forms of hydrocephalus that involve blockages, NPH is characterized by normal intracranial pressure, leading to gradual fluid accumulation and subsequent neurological symptoms.
Symptoms of Normal Pressure Hydrocephalus
NPH presents gradually, resembling conditions like Parkinson’s disease or dementia. Symptoms include gait disturbances, urinary incontinence, and cognitive impairment. Gait issues are the most common, affecting a significant percentage of patients, while cognitive symptoms may include slowed thinking and forgetfulness.
Treatment for Normal Pressure Hydrocephalus
NPH does not respond to medication. Treatment involves draining excess CSF to alleviate symptoms. Diagnostic tests such as a lumbar puncture may be performed to measure CSF pressure and remove fluid. If successful, patients may undergo a ventriculoperitoneal (VP) shunt surgery to regulate CSF flow and relieve pressure on the brain tissue.
Prognosis of Normal Pressure Hydrocephalus
NPH is partially treatable, with shunt surgery improving symptoms in many patients. However, accurate diagnosis is crucial due to its resemblance to dementia. The prognosis varies, and some patients may also have other neurodegenerative conditions alongside NPH.
